Sage Rehabilitation Hospital

SAGE Rehabilitation Hospital and Outpatient Services offers intensive comprehensive and goal directed programs, utilizing a team of rehabilitation specialists; to guide patients along a practical and individualized treatment pathway designed to maximize functional independence sage rehabilitation hospital

Within hours of consultation, a clinical liaison will conduct an on-site pre-admission evaluation and the patient and family will have an opportunity to meet the Sage team and tour the facility.

Sage has a unique opportunity to offer patients a continuum of care that follows them from the initial to final stages of recovery, returning the patient to the home, work or school! With the availability of an inpatient rehabilitation facility, skilled nursing unit and outpatient and day programs, Sage is able to provide each patient a home transition system based on their level of need.

Sage Rehabilitation is made up of compassionate, dedicated staff members. We believe that our employees are our most important assets — and as such, we strive to provide a working environment that focuses on job satisfaction.

We hire specialists such as physicians, nurse practitioners, physician assistants, registered nurses, licensed practical nurses, certified nursing assistants, therapists, dietitians, social workers, chaplains, bereavement professionals, and administrative staff.

Sage Rehabilitation Outpatient/Day Neuro Program offers comprehensive outpatient rehabilitation for people recovering from traumatic and non-traumatic spinal cord injuries.

Outpatient therapy allows patients to continue to participate in intensive rehabilitation while living at home, problem-solve issues encountered in the real world, and immediately practice and apply skills learned during therapy in their home and community.

We serve patients with all levels and types of spinal cord injury, including paraplegia, quadriplegia, spinal cord tumors, transverse myelitis, Guillain-Barre syndrome, multiple sclerosis, myelopathies, spinal cord infarctions and aneurysms.

At Sage Outpatient, the licensed occupational therapists are recognized experts in assessment and treatment of individuals who have been diagnosed with hemianopsia, visual field cut, ocular motor dysfunction, low vision, visual inattention or neglect, visual motor disorder, and visual perceptual disorder.

The occupational therapists can provide training and education on environmental modifications, adaptive devices and assistive technology to enable full participation in their community.

The role of the occupational therapist is to collaborate with the patient to resume daily function to adapt or overcome visual loss. Common diagnoses which may impact the visual system include: brain injury, stroke, brain tumor, various diseases, and aging.
