ZeOmega® Inc. today announced that one of the largest accountable care organizations (ACOs) in America has selected ZeOmega’s Jiva™ population health platform to better coordinate care for 50,000 lives across more than 1,100 network providers. The ACO is part of a prestigious healthcare organization that includes a world-renowned academic medical center in an urban area with more than 7 million residents.

The healthcare organization, which has about 100,000 in-patient admissions and over 2 million outpatient visits across its facilities every year, realized that while its deployment of an industry-leading EHR addressed the needs of its providers within the acute and ambulatory care settings, it needed a more flexible care coordination solution that would enable collaboration across patients, providers and multidisciplinary care teams to fully achieve its Population Health Management (PHM) and value-based care goals.
