What began as a vision decades ago has become our passion: making patient care better by fixing the issues with health record automation – not by making “interoperable systems” that rarely operate and never really work together but by addressing the core issues in a way that produces solutions that are usable, useful and used. The CentriHealth Individual Health Record (IHR) places the care of the individual at the center of the design. Our ability to transform and repurpose all sourced data allows us to deliver individual-centric clinical information in new ways. The IHR is a single, unified and understood health record for each individual in a single database that delivers to providers and individuals personalized and individualized access to the same information, contextualized in the way each user community needs to see it. This allows the entire care team (including patients/members/individuals themselves) to collaborate on each person’s individual care.
