i2i Systems Population Health Management Solutions Selected by Infinity Primary Care to Help Meet Quality Measures

Robust, user-friendly reporting capabilities relieve IT Department burden.

i2i Systems today announced it has been selected by Michigan-based Infinity Primary Care to provide Population Health Management and Analytics solutions.

The private group practice operates 12 physician offices as well as a diagnostic testing center and serves the western suburbs of Detroit. With most of the offices recognized as Patient Centered Medical Homes, Infinity needed easily producible reports.

“We are very forward-thinking when it comes to quality. In addition to our PCMH recognition, we are focused on other quality measures, including Meaningful Use.” said Infinity CMIO, Dr. Robert Carter. “To meet these goals, we needed greater flexibility with our reports.”

To ensure the group was able to meet quality measures and provide the required reporting, Infinity selected i2iTracks, the leading Population Health Management solution, used by more than 1,000 healthcare facilities across 35 states. i2iTracks, helps standardize data from disparate electronic health records, practice management and other systems, simplifying the path to actionable information for an entire patient population. This enables clinicians – and their teams – to proactively, rather than reactively, manage patients, leading to increased staff efficiency and better clinical outcomes.

“We are doing well with financial reporting but have been challenged with some of our quality reports. Even the reports we had required IT involvement and so the demand for reports was outpacing our ability to process the requests,” said Grace Miller, Director of Information Technology at Infinity. “With i2iTracks, we set it up and users can run their own reports and IT is free to focus on other needs and issues.”

“i2i Systems came highly recommended to us and we are very impressed by what we’ve seen so far,” added Dr. Carter.

“We are proud to partner with organizations, such as Infinity Primary Care, that are dedicated to improving people’s lives by delivering the highest quality healthcare possible,” said Janice Nicholson, founder and CEO of i2i Systems. “We’ve designed our solutions to be both sophisticated and user friendly so organizations can easily reap the full benefits of their quality initiatives.”
