Paladina Health and eClinicalWorks Expand Partnership to Improve Patient Health in the Primary Care Setting

eClinicalWorks®, a leader in ambulatory healthcare IT solutions, today announces an expanded partnership with Paladina Health, a provider of integrated primary care to patients across the United States, to include the eClinicalWorks Care Coordination Medical Record (CCMR) population health management platform. Paladina Health delivers primary care focused on population health management, expanding the scope of services delivered in the primary care setting to improve health and lower healthcare costs for patients, employers and insurers.

Paladina Health currently utilizes eClinicalWorks comprehensive electronic health records (EHR) solution. With the addition of eClinicalWorks CCMR, Paladina Health’s care providers will have visibility into a patient’s care across all settings, be more effective in managing complex chronic conditions, and be able to make better, more informed decisions at the point of care. A vendor-neutral, open network securely connects ambulatory EHRs with hospital in-patient systems and payer claim feeds, including CMS, to provide cost and utilization for services delivered to patients. Recently recognized by Frost & Sullivan with its 2014 North America Frost & Sullivan Award for Enabling Technology Leadership, eClinicalWorks CCMR will work with Paladina in meeting its goals as an organization.

Included in CCMR, eClinicalWorks Population Health Management Analytics uses closed-loop analytics to deliver reporting, alerting and messaging capabilities necessary to manage population health. Dashboards can be shown at the community level to determine and monitor adherence of clinical process and outcomes, including medication recalls and chronic disease management. Care coordination and analytics work together for a collaborative care framework.
