Civitas Networks for Health 2025 Annual Conference: From Data to Doing

Civitas’ Annual Conference convenes hundreds of industry leaders, decision-makers, and innovators to explore interoperability, data-driven quality improvement, social determinants of health, public health, multistakeholder governance, and improved health outcomes. Serving as the bridge between “data” and “doing” in healthcare, Civitas’ members and network play a vital role in driving progress.

Attendees come to the conference to build meaningful connections, strengthen existing partnerships, and gain practical strategies to advance initiatives at the national, state, regional, and local levels.

2025 Conference Tracks
To bring our theme of bridging data and doing to life over the course of three days, we’ve designed program tracks that align with this vision. All presentations, breakout sessions, and workshops will be organized within these three focus areas, which reflect the central priorities and ongoing work of Civitas members across the national network.

  • Interoperability, National Networks, Standards, and Technology:
    This track will explore strategies to advance health data interoperability, compare local, regional, and state exchanges with national networks, and examine opportunities for greater alignment across the broader ecosystem. Sessions will include deep dives into standards implementation, as well as highlight emerging technologies such as AI.
  • Promising Partnerships:
    Focused on collaboration, this track will showcase partnerships that drive health data utilities, health equity, population health, quality improvement, and value-based care. Sessions will highlight impactful work among APCDs, CBOs, HIEs, health data organizations, RHICs, and QIOs, emphasizing measurable outcomes from multi-stakeholder, data-driven efforts.
  • Community Information Exchange (CIE):
    Sessions in this track will examine the foundations of strong CIE infrastructure and policies that foster stakeholder engagement, including partnerships with HIEs. Discussions will cover authentic community engagement, data stewardship, system integration, identity and consent management, enhanced care coordination, and sustainability planning—all aimed at helping communities thrive.
