Leading North Texas ACO Selects ZeOmega’s Jiva for Health Mgmt

ZeOmega® Inc. today announced that one of the largest accountable care organizations (ACOs) in America has selected ZeOmega’s Jiva population health platform to better coordinate care for 50,000 lives across more than 1,100 network providers. The ACO is part of a prestigious healthcare organization that includes a world-renowned academic medical center in an urban area with more than 7 million residents.

“We are seeing similar needs for flexible care coordination platforms from other ACOs and health systems who have started their transition to value-based care and now realize that EHRs or other first-generation performance management solutions alone are insufficient to optimize the health outcomes of the people they serve.”

The healthcare organization, which has about 100,000 in-patient admissions and over 2 million outpatient visits across its facilities every year, realized that while its deployment of an industry-leading EHR addressed the needs of its providers within the acute and ambulatory care settings, it needed a more flexible care coordination solution that would enable collaboration across patients, providers and multidisciplinary care teams to fully achieve its Population Health Management (PHM) and value-based care goals.

“This forward-thinking ACO, that demonstrated success under both the Pioneer and the MSSP ACO programs, realized the need for a single platform to support the various value-based care and PHM initiatives it was putting into place. The ACO chose Jiva for its ability to support multiple value-based care initiatives ranging from shared and/or delegated risk to full-risk models and its ability to empower multidisciplinary care teams across multiple care settings, be it the home, ambulatory, acute, or post-acute care settings,” said Nandini Rangaswamy, executive vice president and chief strategy officer for ZeOmega. “We are seeing similar needs for flexible care coordination platforms from other ACOs and health systems who have started their transition to value-based care and now realize that EHRs or other first-generation performance management solutions alone are insufficient to optimize the health outcomes of the people they serve.”

The ACO was attracted by Jiva’s proven interoperability and predictive analytics for better identifying, reaching and treating high-risk patients. The ACO will also be using Jiva’s NCQA accredited disease management capabilities to care for patients with chronic diseases, complex medical conditions, and highly specialized clinical care management.
