Version 3.5 offers several enhancements that allow care managers to outreach and impact more patients in fewer keystrokes

IRVING, TX – Lightbeam Health Solutions, the leader in end-to-end population health management solutions and services, recently announced Version 3.5 of the Lightbeam platform, including the enhanced version of its Care Management Module.

The latest version of Lightbeam’s Care Management Module is the most streamlined yet with new notifications, granular population filtering, and snapshot views of the most actionable data points. The updates greatly benefit the care managers with fewer clicks and steps that promote more time with patients.

The Assessment Builder underwent several enhancements that include the drag and drop question feature, new section headers that allow for quick navigation, the autofill capability to expedite the building of assessments, and more. In addition, the module now has dynamic care plan building functionality, providing recommendations and organizational governance on a patient’s next steps while ensuring the care management team can exercise clinical judgment to customize and individualize care.
