Highlands’ Case Managers are the backbone of your care team. Upon admission to the inpatient program you will be assigned a Case Manager who will be responsible for monitoring the progress of your program and assisting with discharge arrangements. The Case Manager also assists with any insurance issues you may have.

Each week your Case Manager will set up a conference with your physician and the other members of your care team to ensure all aspects of your care are coordinated. A care plan and summary report of this conference will be produced to ensure all of your care needs are being addressed, and that everyone is working toward the same goals. Your Case Manager also serves as a primary contact for you to find out the status of your program and discharge arrangements.
