Our state licensed Physical Therapists are responsible for the planning, implementation and evaluation of our Physical Therapy programs.
Using a comprehensive approach, these therapists provide services to those individuals whose functional mobility is threatened or impaired by developmental deficits, physical injury and/or illness. Our therapists assist patients to achieve their maximum level of functional independence with treatments tailored to each individual’s need.
The provision skillful manner in which nursing care is delivered. The provision of nursing care is supported by licensed practical nurses and certified nurse assistants that are deemed appropriate by the rehabilitative registered nurse. The nursing staff is trained to work with our multi-disciplinary rehabilitative team in maximizing client outcomes and promoting independent living.
By participating in team conferences and relating pertinent information, our Case Manager is able to establish practical and meaningful goals for the patient and family. psycho-social assessment analyzing discharge planning begins upon admission which involves early recognition of post-hospital needs together with an understanding of the social, psychological, cultural, environmental and financial factors which may effect the patient’s recovery.
