Lumeon Launches CPM 4, Extending the Provider EHR to Connect Care Everywhere

New release enables health providers to deliver personalized, patient-centric care plans coordinated across any care team and setting

BOSTON–(BUSINESS WIRE)–Lumeon, the leader in Care Pathway Management (CPM), today announced CPM 4, a new release of its platform that extends a healthcare provider’s EHR system, connecting the dots beyond their four walls to help manage care delivery through their partner network. With CPM 4, providers can take charge, uniting the care delivery process across the patient journey.

While EHR adoption is widespread in U.S. healthcare, the patient journey continues to be fragmented and broken. Providers lack the ability to manage and control the delivery of care across their teams and across many disparate care settings, costing organizations billions of dollars every year.

CPM 4 allows providers to operationalize a longitudinal plan of care for every patient, and importantly, beyond the confines of the EHR system. The care plan dynamically adjusts based on the patients’ preferences, risks and severity, optimally coordinating the most appropriate care resources to deliver better outcomes more cost efficiently.

The CPM platform combines a digital plan of care with intelligent orchestration, automation and patient engagement technologies, empowering care managers to oversee and optimally manage patient tasks. – A sleek, user-friendly interface for care managers, allowing them to track the current progress, status and next steps along the patient pathway. This user interface presents a real-time view of patient milestones and checkpoints, which can be used natively within the EHR.

– As well as existing HL7 capabilities, a new API plug-in framework allows providers to rapidly create their own APIs, connecting the CPM platform to any API-enabled web service or application.

– New intelligent workflow automation capabilities, such an API-aware workflow, enable the patient pathway to automate communication with third-party services, such as CRM, EHR, and PHM systems.
