May 21: New Platform Integrates Advance Directives With Electronic Medical Records

Many patients and their family caregivers do not take the time to prepare an advance directive, which often results in costly, unwanted, or inappropriate treatment for patients at the end of life. However, these errors can occur even when patients have an advance directive because paper documentation can be lost, especially during care transitions, or inaccessible to all members of a patient’s care team.
UMass Memorial Health Care, Worcester, MA, is partnering with Luminat, a company that provides advance care planning products, to test a new platform that integrates patients’ advance directives into their electronic medical records (EMR). Luminat’s system is cloud-based, enabling patients and care teams to access this documentation from anywhere at any time. The easy-to-use interactive platform guides the patient through the advance care planning process to create and manage his or her plan. By being integrated with the EMR, the system can notify providers when patients have created or updated their care plans. The system is secure and HIPPA-compliant, and Luminat also obtains legal validation for the patients’ documentation and works with providers to meet state-specific requirements.
“Luminat’s advance care planning platform integrated with UMass Memorial’s electronic medical records is a first in the industry,” said Tom Valdivia, MD, MS, chief executive officer, Luminat, in a press release. “Up to this point, patients completed the advance care directive on paper and submitted it to their physician to be filed away.
