Luminat’s system is cloud-based, enabling patients and care teams to access this documentation from anywhere at any time. The easy-to-use interactive platform guides the patient through the advance care planning process to create and manage his or her plan. By being integrated with the EMR, the system can notify providers when patients have created or updated their care plans. The system is secure and HIPPA-compliant, and Luminat also obtains legal validation for the patients’ documentation and works with providers to meet state-specific requirements.
