Senior Software Provider Caremerge partners with EHR developer Amazing Charts

Two software companies announced a partnership designed to improve senior patient care while maximizing physician offices’ efficiency.

Caremerge, Inc., a communication and care coordination network for the senior care industry, has partnered with Amazing Charts, LLC, a developer of Electronic Health Records and Practice Management Systems for independent physician practices. The partnership will add to Caremerge‘s Chronic Care Management (CCM) platform.

“We are excited to partner with Amazing Charts, a leader in health IT for independent physician-owned practices,” says Asif Khan, founder and CEO of Caremerge. “Physicians benefit from additional revenue, while patients with multiple chronic conditions receive better care coordination resulting in better health.”

The systems integration will simplify the Medicare reimbursement process for eligible beneficiaries with multiple chronic conditions. The Caremerge CCM platform has allows physicians to visually track how much non-face-to-face time was spent with each patient.
