As part of its care transitions process to eliminate avoidable readmissions, minimize length of stay and boost the patient experience, South Nassau Communities Hospital has enlisted the help of RightCare

RightCare’s full suite of software will be used by South Nassau’s case managers to better meet the individual care needs of patients through a unique, modular software solution that is designed to assess patient risk at the time of admission, ensure the appropriate care plan is in place and seamlessly transition patient information to post-acute care providers. Deeper integration with post-acute care partners furthers South Nassau’s commitment to offering patients an individualized approach to coordinated, quality healthcare.

As part a Performing Provider System (PPS) within New York’s Delivery System Reform Incentive Payment (DSRIP) program, South Nassau can also utilize RightCare to satisfy requirements for DSRIP projects including:

  • An integrated delivery system
  • A care transitions intervention for skilled nursing facility residents

“We treat our patients like family, and we go to great lengths to ensure that we offer the highest quality of care,” said Bill Allison, senior vice president, health policy and planning at South Nassau Communities Hospital. “The individualized approach that RightCare offers helps us to further improve outcomes, while also helping case managers streamline care coordination for increased efficiency. This, coupled with DSRIP requirements that RightCare helps us to achieve, made our decision to utilize their software simple.”

“Our goal is to provide a comprehensive, patient-centered approach by providing case managers the information required to develop individualized care plans for patients. By asking the right questions at the time of admission, we can help predict patient needs throughout the care continuum and determine the most appropriate level of post-acute care,” said Eric Heil, CEO of RightCare. “Our technology, which is used in hospitals across the nation, has made significant strides in helping hospitals reduce costs while improving the quality of the care they provide. We are enthusiastic about working with a hospital that shares our same mission and understands the impact optimized care transitions can have on improving outcomes.”

RightCare is a medical technology company built on one fundamental goal: helping providers improve the continuum of care. It’s the only medical software focused on healthcare transitions using a unique algorithm based on 10 years of clinical and academic research that enables continuous point-of-care decision support, optimizes workflow, and dramatically improves post-acute care transitions.
