South Nassau Communities Hospital Selects RightCare

As part of its care transitions process to eliminate avoidable readmissions, minimize length of stay and boost the patient experience, South Nassau Communities Hospital has enlisted the help of RightCare

RightCare’s full suite of software will be used by South Nassau’s case managers to better meet the individual care needs of patients through a unique, modular software solution that is designed to assess patient risk at the time of admission, ensure the appropriate care plan is in place and seamlessly transition patient information to post-acute care providers. Deeper integration with post-acute care partners furthers South Nassau’s commitment to offering patients an individualized approach to coordinated, quality healthcare.

As part a Performing Provider System (PPS) within New York’s Delivery System Reform Incentive Payment (DSRIP) program, South Nassau can also utilize RightCare to satisfy requirements for DSRIP projects including:

  • An integrated delivery system
  • A care transitions intervention for skilled nursing facility residents
