Villa Medical Clinic in Newburg, Ore., selected Allscripts (Nasdaq:MDRX) Chronic Care Management (CCM) Program to establish ongoing monthly outreach to hundreds of patients who suffer from two or more chronic conditions without adding staff or increasing workloads for current employees.
Villa Medical Center has served its community for more than 50 years and currently provides care services with one physician, three physician assistants and a team of nurses and support staff. The practice sought a way to simplify the rollout of care for patients with chronic conditions through the Centers for Medicare & Medicaid services (CMS) reimbursement program. The practice is in the process of transitioning to Allscripts Professional EHR™ from which it will be able to identify Medicare and Medicaid patients with chronic conditions who could benefit from increased care.
The Allscripts CCM program features services from Allscripts Professional Physicians business unit to assist care providers in reducing the complexity of caring for chronic patients, including advisory services, patient outreach, practice and claims management.
