Wellness Provider Announces New Solution to Deliver Self-management Education Resources

Viverae®, a leader in workplace wellness technology, is proud to announce the addition of Healthwise, a leader in evidence-based health education, technology, and services, into its well-being programs. Healthwise’s patient education and engagement content will soon be integrated into each step of Viverae’s existing industry-leading Disease Management (DM) and health coaching programs to engage members with the information needed to better manage their own health. Consequently, Viverae care managers will be equipped to deliver consistent health content and patient education to enhance care coordination.

When individuals are diagnosed with chronic conditions, oftentimes, they’re left to manage their condition themselves, with little to no assistance. According to research, the skills and knowledge required to successfully self-manage a condition typically involve multiple lifestyle changes, which can leave individuals feeling overwhelmed. To address this challenge, the two companies will work together to offer the Healthwise® Care Management Solution, which includes the Healthwise® Behavior Change Navigator – a tool that is built on scientifically proven behavior change principles. With this solution, Viverae care managers will have access to a powerful resource to assist with their real-time coaching services. Through the system, the coach or clinician will be presented with easy-to-follow coaching tips with suggested education at each stage of the framework, based on proven behavior change techniques.

This five-stage framework consists of the following:

  • Awareness – General information about the condition
  • Learning – Information about what members can do to help manage their condition
  • Motivation – Support or encouragement to help individuals be more proactive
  • Behavior – Ideas about how to take action to better manage the condition
  • Sustainability – Support to sustain healthy actions members are already taking to stay on track
