Cookeville Medical Center in Cookeville, Tenn., selected Allscripts (Nasdaq:MDRX) Chronic Care Management (CCM) Program to more easily manage and improve care for patients with chronic conditions while increasing Centers for Medicare & Medicaid Services (CMS) reimbursements.
Cookeville Medical Center is an existing Allscripts Professional EHR™ client that selected the CCM program to improve engagement with patients for better management of chronic diseases. The CCM program enables Cookeville to offer patient-centered care plans with limited impact to existing operations.
The Allscripts CCM program features services from Allscripts’ Professional Physicians business unit to assist care providers in reducing the complexity of caring for chronic patients, including advisory services, patient outreach, practice and claims management.
