As a patient with multiple diagnoses, frustration was the norm for Judy. Test results were often not shared between practitioners at the Markham Family Health Team and frustration grew. In 1999, the Markham Family Health Team transformed their offices by switching from paper-based records to electronic medical records (EMR). Through this electronic amalgamation of her personal health information Judy’s health care is now managed by a team of doctors and specialists. Dr. Philpott, Judy’s primary health care provider, has eliminated the need for paper work as test results and patient notes are all captured electronically. Dr. Philpott is able to proactively care for Judy and other patients through EMR features by setting electronic reminders, creating medications lists and viewing laboratory results as soon as they become available. Ultimately, Judy’s life has been dramatically changed since improvements to her physical health have led to positively impacting her mental health and her personal life.
