Access Community Health Network

Chicago area’s largest provider of community health care, Access Community Health Network (ACCESS) has nearly 40 health centers, located throughout Chicago, Cook and DuPage counties, and serves as the medical home for nearly 200,000 patients each year. Founded in the early 1990’s, ACCESS is a federally-qualified health center (FQHC) organization accredited by The Joint Commission and strives to improve the health and wellness of our communities.​
