Improving elderly care through sharing EMR

a model for value-based care
THE VISION FOR CARECONNECTION WITH THE WIDER HEALTH ECONOMY
There is a growing need to address the needs of the elderly, especially as thepopulation continues to grow and live longer. Donisthorpe Hall want to bring this issue to light through the use of technology. Maria says “For over 6 years I have been waiting for something like this. We need to bring the issue of elderly care out into the open, there are a lot of homes providing good quality care, and continuously improving standards. Through the use of a shared electronic medical record with the local health providers we will be able to plan the residents care more effectively.” This vision is shared with local GP practices and the CCG. John Snowden, Deputy Practice Manager at Street Lane Practice, Leeds says “We want care homes to be incorporated within healthcare. This can be achieved through the sharing of information with local services to ensure continuity of care and achieve better outcomes for the vulnerable elderly.”
